Coordinated access to provide better outcomes!

Coordinated access is a process through which housing and support services are provided to individuals and families experiencing homelessness or at risk of homelessness. Our process begins with client intake to assess and understand the housing needs, and then we help by matching and referring to available housing.

Getting you the help you need in 3 simple steps.

1. Contact our Intake Coordinator

You can contact intake by email at intakerapidservices@mmf.mb.ca or by phone at our intake line 204 589-1613.

2. You will then be asked to fill out the following forms:

Referral form for referrals from another department/agency.

Or;

Client intake form if you are applying on your own behalf or together with the referring department/agency.

A consent form may be required for someone to receive permission to act on your behalf regarding EIA and other agencies or to help you receive those services.

3. Once we establish contact with you and collect the required information, we can determine the level of need and how Rapid Services can help you.

If you qualify for support, you will be assigned a case manager and a support worker to meet with you and provide immediate and ongoing support as needed.
